  • Carefully review each statement and consider its impact. Make sure your parent or guardian also reads through this document and understands what's expected in PSEO.

  • Digitally sign it when you are done. If you are under 18, a parent or guardian must be with you to complete the form.

  • You will only be able to sign the form once.
